Youth Arise - Bonfire Permission Form 2026

GENERAL INFORMATION

On Friday the 7th August, Youth Arise will be joining a combined Baptist Bonfire Night out at Bookham. We will be leaving from Yass Community Baptist Church (YCBC) at 5.50pm and arriving back at YCBC at 9.45pm. Dinner will NOT be provided, so please ensure your child has something to eat before. For this night can you please ensure that your child/ren have adequate warm clothing, enclosed shoes, that are suitable for muddy conditions, bring a water bottle a camp chair and please bring some snacks to share. I give permission for my child/ren to attend a bonfire night out at Bookham. While I will be contacted as soon as possible in the event of an emergency or my child requiring medical attention, I authorise the leader/s to arrange for first aid, medical treatment or an ambulance as considered necessary. I accept responsibility for payment of expenses associated with any medical treatment required.
